Caradigm delivers interoperability solutions to support the exchange of data across the healthcare community that is necessary for provider population health initiatives.


Since the advent of healthcare reform, the US has experienced rapid growth in the number of Accountable Care Organizations (ACOs) - those within the program under the Center for Medicare and Medicaid Services (CMS) - as well as commercial ACOs. These organizations - comprised of integrated delivery networks, multi-specialty group practices, physician-hospital organizations and health maintenance organizations and other networks - require effective aggregation and communication of information about patients, populations, processes and performance to deliver population health management successfully.

Current solutions for information exchange and population health can pose challenges.

  •    Health Information Exchange (HIE) vendors often don’t have integrated populations health applications or offer    claims analytics
  •    Electronic Health Record (EHR) vendors often cannot aggregate external data, or share aggregated data with other    EHRs
  •    Population health vendors often don’t offer bi-directional integration with EHRs in the community


Caradigm Open Exchange (COE) can help deliver interoperability solutions to support provider population health initiatives. This is achieved by capturing and normalizing information from across the healthcare community, enabling ingestion of data for Accountable Care Organization (ACO) measures and longitudinal patient records and, supporting the handoff across the continuum of care.

Standards from Integrating the Healthcare Enterprise (IHE) IT technical framework (ITI 18, 41, 42, 43, 61), DIRECT and REST architecture are used by COE to exchange data within and across organizational boundaries.

In addition, COE supports on-demand document exchange - electronic Plan of Care, Care Summaries, and other clinical data with community Electronic Health Records (EHRs), allowing for external standards-based queries to generate Consolidated Clinical Document Architecture (CCDA) clinical summaries to be exported and surfaced through Caradigm Intelligence Platform (CIP) entities and used by Caradigm applications.

Caradigm has achieved the ConCert by HIMSS™ with its CIP and COE capabilities.
