BELLEVUE, WA – April 13, 2015– Caradigm, the leader in enterprise population health, today announced a partnership with Eliza Corporation (“Eliza”), the pioneer and recognized leader in health engagement management, to enable true patient relationship management across the continuum of care, allowing providers to improve both patient outcomes and care efficiency.
The new Caradigm Patient Outreach solution leverages Eliza’s expertise in crafting highly effective, multi-channel patient interactions to enable providers and accountable care organizations (ACOs) to reach outside of the care setting and engage patients in health-related behavior change – with the right message at the right time. Interactions are targeted based on patients’ clinical and life circumstances, making them more relevant and compelling, leading to improved patient activation. The system also continues to gain insights about patients based on each interaction, resulting in improvements in both the effectiveness of interactions and patients’ perceptions of their provider organizations.
Caradigm Patient Outreach is designed to help improve patient safety, ensure clinical best practice delivery and maintain regulatory compliance, specifically:

- Transitions of Care – supporting patients through critical progressions between care settings, to reduce readmissions and increase patient satisfaction

- Reaching patients at the right time with the right message, to educate, overcome barriers and gain commitment to complete behaviors
- Enabling patients to complete behaviors when they are most motivated – by providing the ability to schedule or complete them in real-time
